Social Security Cards (SSA issued document showing name and SSN or federal, state, or local government issued document containing the name & SSN may be substituted).
REQUIRED FOR MINORS
Birth Certificate for MINORS. If you do not have Birth Certificates you must provide one of the following: Adoption papers, Custody Agreement, Human Services ID, or certified School Records.
FOR PUBLIC HOUSING (DEERCREEK, SCATTERED SITES)
REQUIRED FOR ALL ADULT HOUSEHOLD MEMBERS:

Failure to provide all requested documents and forms within fifteen (15)calendar days will result in your incomplete application not being processed. You will not be eligible to reapply for 30 days***
